An ingrown toenail occurs when the edges of the toenail grow into the surrounding skin, causing redness, swelling, and tenderness. Common symptoms include pain, inflammation, pus or drainage, skin overgrowth, and bleeding. The big toe is most commonly affected.


Ingrown toenails can be caused by:

  • Improper nail trimming (cutting nails too short or unevenly)
  • Tight footwear (shoes, socks, or tights putting pressure on the toenail)
  • Excessive moisture (sweaty feet softening the skin)
  • Toe injury or trauma
  • Natural nail shape (curved or fan-shaped toenails) 


Nail surgery is suitable for many painful or recurring nail conditions, including:


  • Infected ingrown toenails
  • Damaged or distorted nails
  • Thick or fungal nails
  • Psoriatic nails
  • Recurrent nail infections


Immediate treatment is crucial to avoid complications such as infections, which can spread from the toe to the foot and even to the bone. Prompt care helps protect your overall well-being and prevents more severe issues.


The procedure includes:

  • Initial Consultation: Assessment by our consultant podiatrist to discuss severity and treatment options.
  • Preparation: Normal routine can be maintained; a companion is needed to accompany you.
  • Surgical Procedure: Local anaesthetic is administered, followed by removal of the affected nail portion and a chemical matrixectomy to prevent regrowth.
  • Dressing: A protective dressing is applied. Suitable footwear is recommended. 


  • Post-Surgery Care: You can walk but should wear open-toe shoes and keep your foot elevated.
  • Follow-Up: Three post-operative reviews are scheduled—48 hours, one week, and four weeks after surgery.
  • Returning to Activities: You may return to work the next day if comfortable and able to wear open-toe shoes. Driving is allowed once you can wear closed-in shoes and safely operate a vehicle. Sports should be resumed only after complete healing, typically after 4 weeks.


While generally safe, possible risks include:

  • Pressure Sensitivity: Avoiding pressure on the toe is important during healing.
  • Slow Healing: Some wounds may require additional care.
  • Infection: Minor infections can occur but are usually treated with antibiotics.
  • Nail Regrowth: There is a small chance (about 5%) that the nail may regrow, possibly needing further surgery.

What treatments do we provide?

We provide the 'full' or 'partial' permanent removal of ingrown toenails. A partial nail removal involves taking a small portion of the offending nail off. Nail removal for fungal or damaged nails is also provided.  

What does nail surgery involve?

The procedure involves two injections of local anesthetic into the offending toe. 

This is to completely numb your toe so you don't feel anything during the procedure. 

Then either side or the entire nail will be removed. 

A solution is rubbed into the base of the nail to prevent that part of the nail regrowing.  

Who will perform the nail surgery?

The procedure will be performed by a fully qualified podiatrist who is HCPC registered and a member of the Royal College of Podiatry with years of nail surgery experience both in private practice and the NHS

Step-by-Step Guide to our Ingrown Toenail Procedure

You’ll start with an assessment by our  podiatrist to determine the severity of your ingrown toenail and discuss treatment options. If surgery is recommended, we will explain our procedure and anaesthetic choices


On the day of your nail surgery, you can maintain your normal routine, including eating, drinking, and taking any prescribed medications (unless advised otherwise). You will need someone to accompany you to and from the clinic. The entire process, including preparation and recovery time, typically takes between 1 and 1.5 hours.



  • Anaesthesia: A local anaesthetic will be administered at the base of your toe. This is the only part of the process that may cause slight discomfort, similar to a routine injection.
  • Nail Removal: Once numb, the affected portion of the nail is removed. Partial or total nail avulsion may be performed.
  • Matrixectomy: A chemical is applied to prevent nail regrowth. Any infected skin may also be removed.
  • Dressing: A protective dressing is applied. Bring suitable footwear, as regular shoes may not fit immediately after surgery.


  • Aftercare: You can walk but should wear open-toe shoes and keep your foot elevated.
  • Follow-Up: We schedule three follow-ups—48 hours, one week, and four weeks post-surgery.


  • Work: Depending on the nature of your job, you may return to work the next day if your foot is comfortable and you can wear an open-toe shoe. Manual labour jobs may require additional recovery time.
  • Driving: You can resume driving once you can wear a normal, closed-in shoe and safely operate the vehicle, particularly in making emergency stops.
  • Sports: It’s important to allow your toe to fully heal before returning to sports, especially those that involve direct impact on the toe, such as football. Generally, a recovery period of at least 4 weeks is recommended.


While nail surgery is generally safe and effective, there are some risks to be aware of:
